Enzymatic resistance to betalactams in uropatogenic Enterobacterales

Abstract

The objective of this study was to detect the presence of extended-spectrum betalactamases and carbapenemases in uropathogenic Enterobacterales isolated in the General Hospital "Isidro Ayora", Loja (Ecuador), during the period December 2017-July 2018. Of 323 isolated strains, 90 (27.86%) were extended-spectrum beta-lactamases producers and 6 (1.86%) were positive for carbapenemases; Escherichia coli being the most frequently producing microorganism of extended-spectrum beta-lactamases (77.08%) and Klebsiella pneumoniae of carbapenemases (4.16%). For extended-spectrum betalactamases, the blaCTX-M gene was the most frequent (67.77%); followed by blaTEM (61.11%) and finally, blaSHV, (20.00%); whereas, blaKPC was detected in all the carbapenemase positive strains. The highest percentage of isolation of bacteria carrying both types of beta-lactamases was obtained from outpatients (85.56%) compared to hospitalized patients (66.67%). Significant difference between these enzymes was detected due to sex, type and patient care service, type of extended-spectrum beta-lactamase and microorganism (p ≤ 0.05). Neither strain produced both types of beta-lactamases. There is a high frequency of strains producing extended-spectrum beta-lactamases and a low frequency of strains producing carbapenemases as uropathogens in the studied population
